Patient Forms: If you are a new patient to our office, please visit our patient forms page to complete our new patient forms that will need to be filled out prior to when you arrive at our office. Thank you and please contact our office if you have any questions at all.

Payment Options: For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed.

   
Dental Insurance: At Clear Fork Dental, our number one priority is providing you with the dental care you need most to keep you healthy and happy. To this end, we are not a preferred provider for any dental insurance plans. We believe that this allows us to make the decisions that are best for you. Should you want to work with insurance, we will provide you with all of the information you need to file your claims.
